Doctor Visit Prep
Bring the Patter, Not Just the Complaint
By Cece Lin
We have all been to the doctor and forgotten one of the main reasons we made the appointment in the first place.
Even when we do remember our concerns, it can be hard to explain everything clearly in a short visit. Symptoms, timelines, medications, family history, past test results, and recent changes can all matter. A doctor may not realize how urgent something feels unless we bring the full pattern with us.
For that reason, it can help to fill out a doctor visit prep form before your appointment. Write down your symptoms, questions, relevant history, medications, family history, and anything that has changed. Bring the form with you so you are not relying only on memory while sitting in the exam room.
This form is provided in Word format so you can edit it, add details, remove sections that do not apply, and customize it for your appointment.
The goal is not to diagnose yourself. The goal is to help your doctor understand what is happening, what has changed, and what questions you need answered.
A FREE form to help prepare for your doctor’s visits by noting your previous symptoms, blood test irregularities, family history, etc.
